Add

To create a new Form go to Administration -> Apps -> Modules -> Form Builder. The following options will be presented:

Creating a Form

 1. To create a new form click Add. The following view will be presented:

2. Edit/configure:

Title

Enter an appropriate title for the form. 

Legend Name

Enter an appropriate legend for the form. This is the name of the form as it appears on a page.

Limit Submissions

Check if required.

Submissions Limit

If required, enter an appropriate limit.

Submissions Limit Imposed From

If required, enter an appropriate start date.

Submissions Limit Recycle Period

The ‘Submission Period Recycle Period’ functionality will allow automatic reset of the imposed ‘Submission Limit’ defined by the number of days included within the configuration. Note that the ‘Clear’ option can also be used to clear this limit. The form will be unavailable in the front end of the site once the imposed limit is reached.

Submissions This Period

Easysite tracks the number of submissions. if required, click clear to reset the results to zero.

Prevent form submissions sending emails   

By default, the Form on the page generates a notification email to one or more users. If required check to prevent the form from being generated.

Use in Modules      

If required, check to attach a form to another module. At present this functionality works with:

Allow Paging on Validation Errors          

If the form is paginated the default setting does not allow the user to proceed to the next page unless the mandatory fields on the current one are complete. This option allows the user to move on without completing the mandatory fields on the current page.

Form Type          

Select the appropriate option.

Note: further information on registration forms is available.      

Is Application Form       

If the Forms (Advanced) modules has been installed, check to make the Form an Application form containing pass/fail criteria.

3. Click Next. The following view will be presented:

4. Drag and drop elements to create the form layout. The following elements are available:       

Fields

  • Forms module CAPTCHA element icon Captcha ValidationCAPTCHA is a program that protects websites against bots by generating and grading tests that humans can pass but current computer programs cannot.
  • Forms check box icon v7 CheckboxesThis element creates a question and allows the user to select one or more answers from a list of Check Boxes.
  • Forms module date field icon Date FieldThe Date Field element allows a date to be added to a form, using a calendar interface.
  • Forms module Day month year selector element icon Day Month Year SelectorThe Day Month Year Selector element allows a date to be added to a form, using separate day, month and year dropdowns.
  • Drop down icon Dropdown BoxThis element creates a question and allows the user to select an answer from a drop down list.
  • Forms fieldset element icon FieldsetThis element creates border around a section of the form.
  • File upload icon File UploadThis element adds a browse button to the form, allowing end users to attach documents.
  • Forms module form comments element icon Form CommentThe Form Comments element allows a comment or instruction to be added to a form.
  • Hidden FieldThe Hidden Field element is used to embed Hidden Field within a form, this could be used to contain information for administrators or to include a querystring to capture information such as which asset the end user is attempting to access.
  • HTML5 date field icon HTML5 DateThe Date Field element allows a date to be added to a form, using a calendar interface, and is designed for Web Apps built using HTML5.
  • HTML5 email icon HTML5 EmailThis element allows an email address to be captured on a form and is designed for Web Apps built using HTML5.
  • HTML number element icon HTML5 NumberThis element allows a number to be captured on a form and is designed for Web Apps built using HTML5.
  • HTML 5 range icon HTML5 RangeThis element creates a slider which the end user may manipulate to select a number and is designed for Web Apps built using HTML5.
  • HTML5 URL icon HTML5 URLThis element creates a field allowing the end user to enter a URL and is designed for Web Apps built using HTML5.
  • Mail subscription form element icon Mail Subscription FormThis element works in conjunction with the Email Newsletters module, and allows an end user to subscribe to one or more mailing lists whilst completing a form.
  • Forms module multi tier drop down element icon v7 Multi Tier Drop DownThis element adds a multi choice element to a form, allowing the end user to select from a series of tiered lists.

5. Click Save, and then Finished.
